The nomination application can be completed by a nominator or nominee and will take between 20-30 minutes to complete. You may leave and return to the application page at anytime. You may print the PDF application below then return to complete the form on-line. Please scroll down to read all of the instructions before you begin. The deadline for nominations is July 31, 2016.

Nomination Submission Guidelines

  • Preferred submission is electronically via form on this site to or via fax to 407.898.2828 with the subject: Women of Distinction Nomination
  • Nomination must include a one-page letter of introduction and two letters of recommendation of the nominee.
  • Each of the three questions located in the nomination form should be answered to the best of the knowledge of the nominator or nominee without leaving any question unanswered.
  • A high-resolution head-shot of the nominee must accompany nomination submissions and will be used for the Awards Ceremony- one side of image must be a minimum of 1,000 pixels.
  • All entries must be typewritten.

Eligibility Requirements

  • Must live or work in Brevard, Lake, Orange, Osceola, Seminole or Volusia Counties.
  • Exemplify extraordinary civic, professional and/or philanthropic commitment and achievement.
  • Be a role model for girls, demonstrating a commitment to the advocacy for improving the lives of women and/or girls.
  • Demonstrate initiative, integrity and leadership characteristics.
  • Nominees are NOT required to have been or currently be a Girl Scout.

Selection Process

Nominations will be sent to another Girl Scout Council in the country and reviewed by a selection committee based on the criteria for each of the categories. All nominees will be honored during the event. Winning recipients should be present to receive their award and be able to participate in post media and promotion of the event. No incomplete nominations will be considered.
